An adult woman died after being hit by an ION light rail train in Waterloo late Saturday night. Emergency crews were called to Dutton Drive near the ION Operations, Maintenance and Storage Facility at about 11:20 p.m. on August 29, 2026. Despite attempts to save her, the woman was pronounced dead at the scene.
Waterloo Regional Police say their General Investigations Unit looked into what happened and have not laid any charges. The woman’s name has not been released.
The crash happened near the rail yard and maintenance facility where ION trains are stored and serviced. It is the latest in a series of pedestrian collisions involving the ION system since it launched in 2019. Over the years, at least six pedestrian incidents have resulted in deaths or serious injuries, raising concerns about safety along the rail corridor, especially in areas away from station platforms.
According to Waterloo Regional Police, the investigation is complete and no charges will be laid. The ION system is operated by Keolis Grand River under a contract with the Region of Waterloo.
This fatality adds to a troubling year for road safety in Waterloo Region. Police reported a 26 per cent increase in fatal collisions in 2025, with seven pedestrian deaths out of 20 total traffic fatalities.
